The purpose of gathering customer information at the time of application of insurance is to determine the premium and appropriate  insurance coverage according to the risk. The address is collected in the event of a claim to identify the client. We collect your name, date of birth, address and home phone number, dates of travel and destination and residency .

We may need to collect from your attending physician and family physician past medical history, records, specialist referral reports, emergency room assessment, clinical history and progress notes of hospital admission. Operative notes, pathology and x-ray investigations and discharge summary. Your consent is requested by asking for the your signature on the declaration form at the time of purchase of your insurance. At the time of a medical loss, your authorization to release medical information is requested. Should you not be in a position to provide authorization, any person acting on your behalf can provide authorization. This authorization is imperative in order to provide assistance to you and your family and to determine liability and arrange payment directly with the medical facilities. In the event of an accident or sickness where you may be hospitalized, our ability to assist will be limited by the amount of information we are able to obtain. Quick authorization to release this information will make a difference on the outcome of the service provided.

Information collected is solely for the purposes related to the determination of liability under the insurance policy. Once the information is collected and used it is kept in secure storage for a period of time as required by law.

Information collected is assumed to be accurate based on the information provided to our company at the time of application of the insurance and at the time of loss. Information is only altered if inaccuracy has been identified. Quality standards are in place to ensure the recording of the information is accurately documented.

Information collected for the purposes of adjusting your claim and is kept secure by the insurance companies specifically.
