Standardized descriptions

Alzheimer's disease As evidenced by loss of intellectual capacity involving impairment of memory and judgement resulting in the reduction in mental and social functioning and requiring supervision for activities of daily living
Blindness As evidenced by permanent loss of sight in both eyes with corrected visual acuity of less than 20/200 or a field of vision in both eyes less than 20 degreesÂof less than 20/200 or a field of vision in both eyes less than 20 degrees 
Coronary Artery Bypass Excludes non surgical techniques like balloon angioplasty or laser embolectomy
Deafness As evidenced by permanent loss of hearing in both ears with an auditory threshold of more than 90 decibels 
Heart Attack As evidenced by ECG changes and elevation of cardiac enzymes 
Kidney Failure As evidenced by irreversible failure of both kidneys which necessitates treatment by regular dialysis or transplant 
Loss of Limbs As evidenced by the total and permanent severance of two or more limbs from above the wrist or ankle joint 
Lou Gehrig's disease As evidenced by a licensed and practicing neurologist's report stating the unequivocal diagnosis of motor neuron disease 
Major Burns As evidence by third degree burns covering at least 20% of the body
Major Organ Transplant on Waiting list Implies the payment of benefits once the insured has been accepted to a recognized organ transplant program's waiting list 
Major Organ Transplant Organs include; heart, lungs, liver, kidney and bone marrow 
Multiple Sclerosis (MS) As evidenced by the unequivocal diagnosis of MS confirmed by modern investigative techniques. The MS must be characterized by at least two episodes of well-defined neurological abnormalities lasting for a continuous period of at least six months (180 days) 
Occupational HIV Infection Accidental injury during the course of the insured's normal occupation and evidenced by standard testing protocol 
Parkinson's disease As evidenced by primary idiopathic Parkinson's disease which includes two or more of the following symptoms: rigidity, tremors or bradykinesis 
Stroke As evidenced by neurological
